PAYMENT POLICY

For Eye Exams and procedures, co-payments are due at the time of service.  If insurance does not cover the services provided in full or if there is a deductible, patients are responsible for the remaining balance.

For cosmetic procedures, payment in full is due at the time of service.

There is no charge for a cancellation at least 72 hours prior to the appointment.

Patients will be charged a fee for last minute cancellations of cosmetic procedures.
